Our Disability Support Workers help bring purpose, skills training, and personal development for people with disability within inclusive environments.

Community Connections

C2A Community Connections hubs continually evolve and develop programs in line with our participants’ goals, however, one thing has remained constant: our ongoing passion for understanding the needs of people with disability, their carers, and families.

Community Connections’ Disability Support Workers assist with and develop engaging programs such as urban adventures, Social and Community activities, hobbies and crafts. It’s all about allowing our participants to live the best lives they can, in a creative inclusive environment.

Our Disability Support Workers providing complex and high intensity supports are highly skilled and trained in working with complex needs.

C2A provides ongoing professional development and training to meet our participants’ goals.

Our Disability Support Workers support a range of daily living activities like dressing and grooming, personal care, assisting with communication devices, supporting the use of mobility aids, medication management, diabetes management, seizure management, mealtime management and more.

We also implement Positive Behaviour Support Plans created by Behaviour Support Practitioners and the participant.
